Reach the fragment, then interact
Land beside the fragment and interact to collect it. Passing through its position in midair is not the pickup action. If the prompt does not respond, finish your current attack and try again while standing nearby.
Four fragments of the same kind grant one permanent upgrade: health fragments add 20 base HP per set, and mana fragments add 20 base MP per set. The two kinds count separately. The directory contains 24 of each kind, enough for six sets each.

Open a location to see its room and pickup instructions. When a hidden passage is listed, follow its wall, floor or lever link before returning to the fragment. These links describe the local opening; they do not specify a complete route from a save room.
